3717-1-02.4(C)(15): Critical – Employees were not informed in a verifiable manner of their responsibility to report health information. This was corrected during the inspection by providing the necessary documentation.
3717-1-02.4(C)(17): No written procedures for responding to vomiting or diarrheal events were available. The facility was provided with the required procedures during the inspection.
3717-1-03: Food Safety – Inspectors observed prepackaged sandwiches and cheese sticks in the walk-in cooler that were discolored and held at an unsafe temperature of 70 degrees. The affected food items were discarded.
3717-1-04.6(B): Critical – Utensils and food-contact surfaces were not sanitized at the required frequency due to the disconnection of the three-compartment sink. The facility was instructed to purchase a plumbing permit and, once approved, reinstall the sink to ensure proper cleaning and sanitization every 24 hours.
3717-1-04.1(H): Nonfood-contact surfaces were not easily cleanable, with untreated wood being used for shelving. The facility was advised to apply paint or sealant to the wood.
3717-1-04.2(I): No sanitizer test kit was available on-site.
3717-1-04.2(B)(1): Repeat – The three-compartment sink for manually washing, rinsing, and sanitizing utensils had been disconnected and moved. The facility needs to purchase and approve a plumbing permit before reinstalling the sink.
901:3-4-16(C): The facility did not comply with Person in Charge (PIC) certification requirements.
