The inspections were from March 1 through March 8.

Location: 1080 N Bridge St, Chillicothe
Violations:
- Accumulation of debris on floors and walls in the dishwashing area and around the ice machine (Repeat)
- Light intensity less than ten-foot candles in required areas (Repeat)
- The current FSO license is not displayed

Location: 1071 N Bridge St, Chillicothe
Violations:
- Employees not informed in a verifiable manner of their responsibility to report information about their health (Critical)
- No hand towels are available at the hand sink in the kitchen
- Utensils stored improperly on the steam table
- Shelving by the drive-thru window in disrepair (Repeat)
- Ceiling vents throughout the kitchen with a build-up of dust and debris (Repeat)

Violations:
- Salad toppings not being cold held at the proper temperatures (Critical)
- Prep cooler unit holding salad toppings that were not holding temperature

- No hand soap available at the hand sink by the dish machine (Critical)
- No chlorine test kit is available for testing the sanitizer concentration of the dishwashing machine

Violation:
- Observed automatic hand sink in the bar area with a dead battery.
Corrective Action: The person in charge (PIC) said the battery was going to be delivered that day.

Location: 741 Safford, Chillicothe
Violations:
- Bulk food for customer self-service not properly labeled.
- A food packaged in the facility not labeled.
- Dispensing tube on cream dispenser not cut properly.
- Non-food contact surfaces of equipment are unclean.
- Accumulation of debris under sink at drink station.
Corrective Action: Reviewed cleaning and labeling requirements with PIC.

Location: 774 N High St, Chillicothe
Violations:
- Facility does not have an employee with manager certification in food protection.
- TCS foods not being cold held at the proper temperature.
- Non-durable equipment observed.
- Insufficient air gap between the flood rim and the water supply inlet on the pop machine discharge line in the lobby.
- Accumulation of debris on floors and walls located in prep and dishwashing area.
- Light intensity less than ten foot candles in required areas.
- Ventilation system not maintained.
Corrective Action: Reviewed food safety certification, proper air gap, cold holding temperature, and equipment repair with the PIC.

Location: 1150 N Bridge St, Chillicothe
Violations:
- Cutting boards across from the grill that are unable to be easily cleaned.
- The steam table with the knobs broken, making it difficult to tell if the warmer is on or not.
- Warewashing machine does not automatically dispense detergent sanitizer.
- Multiple food storage containers with the date marking sticker left on the outside of the container from previous uses.
- Water leaking from the steam table.
- Debris build-up around floor drains in the dishwashing area.
Corrective Action: Reviewed violations with the PIC and repeat violations may result in an administrative hearing.
